RN Case Manager

  The candidate will Case Manage approximately 10-15 patients/families. Some of the patients will be in an Assisted Living Facility or Personal Care Homes, and some will be in private homes. North West Houston, includes Spring, Cypress Station, The Woodlands, Westfield, Humble, Kingwood area. 

Previous experience a plus, but if you have a servant’s heart, a call to ministry, and Nursing experience, we can teach you hospice nursing.

CNA

We have available per visit and full-time position for a Hospice CNA in all four quadrants (NW,NE,SW,SE) of Harris Co. Our patients are in Assisted Living, Personal Care Homes and private residences. Weekends shifts available.

Triage/On Call Hospice Nursing

Full-time after-hours Triage/On call. Friday  after hours starting at 5:00 pm on Friday to 8:30 am Monday . Be part of a team with Admin On Call. You will be a W2 employee with travel expense and tolls reimbursed. Benefits.

RN Case Manager– immediate opening in the  North West and South East Harris County area. The NW includes Cypress Fairbanks, Willowbrook, West Road, Katy, Tomball and Magnolia area. The SE includes Baytown, Pasadena, down I 45 to Texas City.

Hospice Nursing

Allows the clinician to use all of your nursing skills, people skills, and spiritual skills. You will provide end of life care/ palliative care to His children as they prepare to return home to be with our Lord.

Volunteer

Grace is always looking for compassionate people to visit our patients, providing support to those who are nearing the end of their earthly lives. This companionship takes the form of weekly or twice-monthly visits to a patient’s home which could be their house, an assisted living or memory care facility, or a personal care home in a neighborhood. Listening is a big part of providing companionship, but you might also read or play a game or take your patient outside for a bit of fresh air. If they are unable to communicate, just holding their hand may be in order.

Volunteers attend a 3 ½ hour training held at our Kingwood office and are also expected to have a background check and annual TB skin test that is required of all people working in a healthcare setting – all of which are provided by Grace Hospice. We serve patients all over Houston and will endeavor to find a patient that is as close to your home or job as possible.

Our volunteers’ lives are blessed when they become a blessing to patients and their families, who often write how much their loved ones’ lives were enhanced by a volunteer’s attention.

  • Bill

    Bill was in the bed but awake and glad to see me. We did the “how’s it going?” drill and then we were quiet. I was looking at his wedding photo and then I asked if he remembered his wedding night. I said I wasn’t asking about the details but how he felt – excited, nervous, scared?  He grinned and said all of the above! He told me they went to Biloxi after the reception. He talked about many memories after that for almost an hour.  He said the pain in his side had eased up quite a bit.

  • Carl

    Carl looked great and seemed to be in good spirits. He looked like he had just been showered and had on fresh clothes. I found him in the activity center.  I re-introduced myself to Carl. He said he remembered me. I wheeled him from the activity room to the library where it was quieter. I asked him a lot about his life and he in turn asked me about mine. He loves books by Michael Creighton and books about the history of Japan since he had lived there.  I told him I would be back in a couple of weeks and I would try to bring some books he might like.  I think he loved the one on one.

  • Martha

    Spent the first 20 minutes reading and praying with Martha. She was thrilled I was back and of course happy to see her new supply of chocolate!  Told her I would be return on Tuesday.

  • Mr. Richards

    Mr. Richards was in great spirits. I sat next to him as he sat in the recliner in the living room. We listened to some zydeco music that I had uploaded on my phone that reminded him of his wife and time in Louisiana. He smiled and laughed as we talked. He remembered me also which was nice. I think my visit had a significant impact.

Grace Hospice of Texas was founded in 2003 by Romeo Villarreal, RN, and Kim Thibodeaux, RN, who are still very active in the company.  The staff consists of people who believe that care of our patients is more than a job and is a ministry performed with a servant’s heart for God’s children on their journey home.  Our logo symbolizes the grace bestowed on us by God through the sacrifice of our Savior Jesus Christ.
